UPDATE (4/4/16): Due to a few 'bugs' that need fixing, the interactive map is temporarily offline until further notice. Please use the list by state until we can fix the problem.
So many people have chipped in, but we've had two absolute rock stars who have gone above and beyond to help create this list. Marcus Tempey has single-handedly verified ten states plus Puerto Rico. Edie Hoffmann has verified five states, including New York, which has 148 medical professionals (second only to Massachusetts).
We will continue to update the map as the addresses of the remaining 1,400 medical professionals are verified. This map is searchable by zip code, or simply zoom in to your area and hover over the dot to see the name, address, specialty, and in some cases, the telephone number and address of the office. As a reminder, the following physician information was generated using publicly available information and data released in April 2015 by the Centers for Medicare & Medicaid Services (CMS). This information is not meant to imply any physician or prescribing agent is familiar with or supporting of the Sinclair Method for treating alcohol use disorder. However, every medical professional on this list has been verified by CMS as having prescribed naltrexone in 2013 within Medicare Part-D. This information was legally obtained using publicly available data, therefore we will not accept requests to remove physicians from this set of lists. Click to view the information in list form.
